指南制订的目的: 前交叉韧带(ACL)是膝关节内最重要的稳定结构之一,同时也是运动过程中最易损伤的结构。ACL损伤多见于年轻或爱好运动的人群,这与我国群众体育参与度的提高密切相关。目前,ACL损伤的标准治疗方式是ACL重建。然而,该手术仍有5~15%失败率,且存在患者重返运动率低,远期继发骨关节炎的不足。为了解决上述问题,近年来,不移除原有韧带,只进行原位修复的ACL初次修补又重新开展。但ACL初次修补与ACL重建存在理念与技术上的根本不同,且无论国内、国外尚未就ACL初次修补达成一致性、具有约束力的指导指南或共识。因此,为规范ACL初次修补的临床应用,由中华医学会运动医疗分会下肢运动创伤学组、中国医师协会运动医学医师分会发起,采取改良Delphi法制定专家共识。
Purpose of the guideline: Anterior cruciate ligament (ACL) is a crucial stabilizing structure within the knee joint and is particularly susceptible to injury during sports activities. ACL injuries are predominantly observed in young individuals and/or those who actively engage in sports, which can be attributed to the growing popularity of mass sports participation in China. Currently, ACL reconstruction serves as the standard treatment for ACL injury; however, this procedure still exhibits a failure rate ranging from 5-15% and presents challenges regarding patient return to sports and long-term development of secondary osteoarthritis. In recent years, ACL primary repair without ligament removal and only in situ repair has been reintroduced to address the aforementioned issues. However, it is important to note that ACL primary repair differs fundamentally from ACL reconstruction in terms of concept and technology. Currently, there is no consistent guideline or consensus on the clinical application of ACL primary repair both domestically and internationally. Therefore, to standardize its clinical use, Lower Limb Sports Traumatology Group of Chinese Society of Sports Medicine Branch of the Chinese Medical Association and Sports Medicine Society of Chinese Medical Doctor Association have initiated an expert consensus development using a modified Delphi method.
