指南制订的目的: 创伤(Trauma)是全世界的重大公共卫生问题,据WHO相关数据,创伤目前是发达国家45岁以下人群第1位的死亡原因,是发展中国家人群死亡的主要原因之一。在中国,创伤是城乡居民五大死因之一,2017年创伤导致的死亡人数超过70万,直接医疗花费达650亿元,导致的死亡和残疾给社会和家庭带来沉重负担。大量研究结果证实,创伤救治体系及创伤中心建设能够改善创伤救治效果。国家卫生健康委于2018年发布了《关于进一步提升创伤救治能力的通知》,要求加强以创伤中心为核心的区域性创伤救治体系建设,提升创伤救治相关专科医疗服务能力,并提出了16项创伤中心医疗质量控制指标。质控指标对创伤中心建设发展起到引领作用,为进一步加强创伤救治的医疗质量管理,规范临床诊疗行为,促进医疗服务的标准化、同质化,国家创伤医学中心医疗质量控制专业委员会对上述16项指标进行了细化和更新,制定了新版19项质量控制指标,包含名称、定义、计算公式、意义、质控对象等,供各级创伤中心在医疗质量管理和控制工作中使用。
Purpose of the guideline: Trauma constitutes a significant public health issue on a global scale. According to data from the World Health Organization, trauma has become the primary cause of death among individuals under 45 years old in developed countries and one of the main factors contributing to mortality in developing countries. In China, trauma ranks among the top five causes of death for urban and rural residents. In 2017, fatalities resulting from trauma exceeded 700,000, with direct medical expenses amounting to 65 billion yuan. The mortality and disability engendered by trauma impose substantial burdens on society and families. Numerous studies have affirmed the pivotal role of trauma care systems and trauma centers in enhancing trauma treatment outcomes. In 2018, the National Health Commission of China issued a notice titled "On Further Enhancing Trauma Treatment Capability." This notice calls for strengthening the construction of regional trauma treatment systems centered around trauma centers, improving the capabilities of specialized medical services related to trauma treatment, and introducing 16 medical quality control indicators for trauma centers. These quality control indicators have played a guiding role in the development of trauma center construction. In a bid to bolster the management of medical quality in trauma treatment, standardize clinical diagnostic and treatment behavior, and promote the standardization and homogenization of medical services, the Medical Quality Control Committee of the National Trauma Medicine Center has refined and updated the aforementioned 16 indicators. Consequently, a new set of 19 quality control indicators has been established. This updated set encompasses names, definitions, calculation formulas, meanings, and quality control subjects, providing a resource for trauma centers at all levels to utilize in their medical quality management and control.
